EvergreenHealth has been working on a project to improve the information in the provider profiles on the EvergreenHealth website, where prospective patients are most likely to search for providers. We want to make sure our profiles take advantage of best practice and make it easy for prospective patients to find an EvergreenHealth provider for their care. We are extending this initiative to update the profiles to all EHP participants.
The new profiles will emphasize what patients want to know first and foremost – your experience and your clinical expertise. Personal information will add another dimension to help prospective patients connect with a provider. Quality and patient satisfaction data will be added down the road.
We need your help! If you haven’t already done so, we need you to complete the attached questionnaire. The information will be used to build our new online profiles. We prefer to get them back as electronic files, but we will accept handwritten ones if they are easy to read. Email completed questionnaires to gneubert@evergreenhealth.com. We need to have all questionnaires returned by Sept. 1 to be part of this profile enhancement.
