What We Accomplished in 2014

2014 was a groundbreaking first full year for EvergreenHealth Partners. I’m going to quickly recap our many accomplishments, but first I want to say thank you. None of these accomplishments would have been possible without your interest in the EHP program, the work of you and your office staff to incorporate the EHP reporting program into your daily work flow, and your commitment to providing the best health care and value to our community.

EHP Keeps Growing:  We continued to add new participants to our program throughout 2014, bringing our totals to 535 providers in 85 practices (54 independent, 31 employed) covering 44 specialties.  

Contracts:  We signed four contracts in 2014 (Cigna, Regence, the Puget Sound High Value Network and the First Choice EvergreenHealth employee plan) and launched two of them (First Choice on Jan. 1 and Cigna on July 1). We ended 2014 with a total of 11,947 covered lives; the addition of the Aetna, Regence and High Value Network plans will add even more in 2015, starting with 6,852 for Aetna alone.

Nurse Care Coordinator:  We hired a fulltime care coordinator – Debi Jefferson, RN. Debi is your central link to organize care and services for those patients who have the potential to become high risk due to uncontrolled chronic illness.

Quality Coordinator:  We also hired a fulltime quality coordinator – Sara Rutherford. Sara is your point of contact for any questions related to quality metrics, targets and strategies.  

Quality Dashboards:   EHP established more than 45 specialty-specific quality dashboards, each displaying results for the quality metrics which are reported for each provider in that specialty. Practice managers are able to view the dashboards and results for their specific providers, with the capability to drill-down on the results to the patient-level detail for each of the metrics.

2014 Metrics:  As of the end of the year, we’re pleased report that 86% of EHP's participating providers had achieved “green” status (reported on 80% or more of their quality initiatives) while 3% had achieved “yellow” status (reported on 50%-79% of their quality initiatives).  

Interest in EHP:  The launch of our clinical integration network generated media coverage both locally and nationally, along with speaking engagements to talk about the formation and goals of EHP.
