- place of service
- rendering physician name

How will I know if there is an issue with my claim feed?
An EHP representative will be monitoring the claims feed status on a regular basis and will notify you or your practice manager should an issue arise. If at any time you have questions or issues, contact Tiffanie Schaefer (tmschaefer@evergreenhealth.com ) and she will route your question to the appropriate person to get you the help you need.
How are the EHP Quality Measure results calculated from my claims data?
The claims information is used specifically to calculate results for the EHP Ambulatory-based measures that are denoted with a “PQRS” (i.e. PQRS 226). These are measures for which CMS has created specific rules for calculation based on claims data. EHP is using these rules as a guide for calculating the results.
